Every day we focus on transitioning clients to the greatest level of independence while achieving the highest quality of life possible. The end goal is a successful reentry at home (family), work and community roles this a process that is unique to each client.
Often clients in our program make significant gains in functional areas (behavior, activities of daily living ADL’s , etc). It is important for us to celebrate these accomplishments with clients (family, etc) however this is also an opportunity to remind all invested parties that we refrain from voicing overly optimistic expectations because we understand how fragile life is. In all of our relationship there is a real risk associated with an unfulfilled promise and expectations among the client, family, guardian / conservator, nurse case manager and payer all of whom must be aligned for short and long term goals. A genuine and sincere commitment to quality and cost is conveyed and understood.
